Elite Dental Insurance Plan
is a great way to
save money on routine dental
care. This plan includes
both insured dental coverage and
discounts. Although you
can use any dentist with this
plan, if you use a network
dentist - your benefits will go
much further. |
| The
Elite dental insurance plan pays 100%
for Exams. Cleanings, Bitewing
X-Rays and Filings regardless of
the dentist you use. If
you use a DenteMax network
dentist, the plan also provides
discounts of 20-50% on other
dental procedures such as root
canals, crowns, extractions,
dentures and orthodontics.
There are approximately 85,000
DenteMax network dental
providers so there is a good
chance that your dentist already
accepts this plan |

you use a out-of-network
dentist, the claim payment is
based on the same amount that
would be paid if you were to use
a network dentist. |
| Note:
When using a network dental
provider, they are contracted to
submit the insurance claim for
covered dental procedures on
your behalf. When using an
out-of-network dentist, you may
be required to submit the claim
yourself and the dental office
can help you complete the form.
Claim forms are included with
your dental insurance policy. |

Elite Dental Insurance plan
utilizes the DenteMax dental
provider network. DenteMax
is a subsidiary of Blue Cross
Blue Shield of MI and has over
85,000 dental providers
nationwide. |
| What
is the annual benefit maximum? |
| The
insured coverage annual maximum
is $1,000 per person. Other
services have network discounts do
not go towards this amount and
the discounts can be used as
often as needed. |
| How
soon can I use the dental plan? |
| Your
plan will become effective on
the 15th of the month if you
apply between the 1st and 14th
of the month. If you apply
on the 15th of the month or
later, your plan will become
effective on the 1st of the
following month. You will
receive a temporary ID card and
policy by email after you
apply. Your permanent ID
cards will be sent by mail. |

there age limits for this dental
plan? |
| There
are no age limits and everyone
is accepted as long as you live
in a state where the plan is
available. |

| Aside
form the insured dental
coverage, this plan also
includes discounts on: Diabetic

services, prescriptions and

are my payment options? |
| You
can pay for your plan monthly
using a credit card or automatic
bank draft. |

my dentist recognize the Elite
Dental Plan? |
| This
dental plan utilizes the
DenteMax network of providers.
When calling to schedule an
appointment, simply mention that
your plan uses the DenteMax
network of providers and fee
schedule. Additional
information for the dentist is
available on your ID card.
The provider can also call the
customer service number. |

are my dental claims reimbursed? |
| The
network dentists are contracted
to file your dental claim form
with the plan
administrator. Dental
services that are covered 100%
are paid directly to the network
dentist. The network
discount is applied to other
services and you would then pay
the remainder to your dentist
office. |

much are the discounts for the
dental procedures that are not
covered 100%? |
| The
discounts range from 20-50% and
vary by zip code. For
specific procedure discounts
when using a DenteMax dental
network provider. |

any provider! However,
benefits are calculated
using a Maximum Allowable
Charge. Maximum Allowable
Charges are limitations on
billed charges and are
based on fees paid to
Network Providers in the
geographic area where the
expenses are incurred. For
Out of Network Providers,
members are responsible
for amounts charged by the
provider that exceed
benefits, and may be
required to remit payment
at the time of service. |

Members will enjoy
discounts, on average 20%
to 50% off standard fees,
on other covered dental
procedures when you see a
participating Network
Provider. Members who see
an Out of Network Provider
will not be entitled to a
discount. Discounts will
vary by provider and
